What is the NHS Discharge Medicines Service (DMS)?

The DMS is a free NHS service available to patients in England who have been discharged from hospital within the last four weeks. Our specially trained pharmacists will review your discharge medications, ensuring you understand how and when to take them correctly, and check for any potential issues or discrepancies with your usual medications.

Who is Eligible?

If you’ve been discharged from the hospital within the past four weeks, you could be eligible for the DMS at Wilstead Pharmacy. Please check with our pharmacy team in store or contact us.

How to Access the NHS Discharge Medicines Service

  • Referral from your hospital: Your hospital that has discharged you will need to send details of your medications to the pharmacy.
  • Speak to Our Pharmacist: Our pharmacist will review your medications, match it against your prescription issued after the discharge, discuss any concerns, and offer guidance on safe and effective medication management.
  • Follow up: We will follow up with you to check that all is going well.
